6706b99828 Delete remnants of registrar-level credits
We never fully used this stuff but definitely no longer use it following our
recent billing refactor. It's confusing to retain all of these entities and
commands given that none of them are actually used by anything.

64986442bc Allow cert hash and fix array out of bound problem in OT&E command
Allow specifying certificate hash other than certificate file. This makes things easier when only setting up EAP registrars. The certificate hash can be easily pulled from existing registrars (SUNRISE, GA, etc) with automation.

Also fixes a bug where we always expect the registrar name + phase string to be at least 7-character long.

98a61b8181 Add the ability to require premium fee acking for a registrar
When enabled for a registrar, all EPP operations on premium domains that have
costs (e.g.  creates, renews, transfers) will fail unless the EPP fee extension
is used to explicitly ack the amount of fee as part of the EPP transaction.

This ack is required regardless of whether premium fee acking is required at
the registry level. No data migration is necessary since false is the desired
default for this new attribute.

This CL also contains some slight refactoring of static utility methods used to
perform fee verification; there was short-circuiting at call-sites in two
places when what was really needed was two methods, one implementing additional
functionality on top of the other, and calling the inner method in the places
where short-circuiting had previously been necessary.

c0f8da0c6e Switch from Guava Optionals to Java 8 Optionals
This was a surprisingly involved change. Some of the difficulties included
java.util.Optional purposely not being Serializable (so I had to move a
few Optionals in mapreduce classes to @Nullable) and having to add the Truth
Java8 extension library for assertion support.

35624ace08 Require currency specific billingAccountId for registrar
For the currency specific billing account ids to work properly, any REAL registrar that is allowed to register domains in a certain TLD must have a billing account id for the currency of that TLD.

d7e2009ddf Add billing account map to Registrar entity
A CurrencyUnit-to-BillingAccountEntry map is persisted in the Registrar entity. It provides flexibility for billing systems that assign different account ids for accounts under different currencies of the same registrar.

4813ed392b Rename "clientIdentifier" to "clientId" almost everywhere
It's best to be consistent and use the same thing everywhere.  "clientId" was
already used in more places and is shorter and no more ambiguous, so it's the
logical one to win out.

Note that this CL is almost solely a big Eclipse-assisted refactoring. There are
two places that I did not change clientIdentifier -- the actual entity field on
Registrar (though I did change all getters and setters), and the name of a
column on the exported registrar spreadsheet. Both would require data
migrations.

Also fixes a few minor nits discovered in touched files, including an incorrect
test in OfyFilterTest.java and some superfluous uses of String.format() when
calling checkArgument().

c458c05801 Rename Java packages to use the .google TLD
The dark lord Gosling designed the Java package naming system so that
ownership flows from the DNS system. Since we own the domain name
registry.google, it seems only appropriate that we should use
google.registry as our package name.

This change renames directories in preparation for the great package
rename. The repository is now in a broken state because the code
itself hasn't been updated. However this should ensure that git
correctly preserves history for each file.
